- The front-side contains a graphic image which represents Jerzy Urban in a form of a ballot box with holes on both top and the bottom, and two ballots with captions "yes" and "no" respectively.
The back side contains a rhymed text which calls to boycott the 1985 elections: "Government will feed itself, the Sejm will elect itself, in such shady business don't take part yourself".

Na odwrocie ulotka zawiera rymowankę, która nawołuje do bojkotu wyborów z 1985 r.: "Rząd sam się wyżywi, Sejm się sam wybierze, nie bierz udziału w takim procederze."
