- Front-side contains a graphic image, which represents a medal with caption "for the Przemyk case, 1984". The image on the back-side represents two Warsaw street plaques on of them crossed out, with caption "approved by the mayor following the decision made by Provincial Court". Jezuicka street refers to a police office where Grzegorz Przemyk was killed. Szucha Alley refers to the location of the main Gestapo prison in the occupied Warsaw.

- Na przodzie, ulotka zawiera grafikę, która przedstawia medal z napisem "za sprawę Przemyka, 1984". Grafika na odwrocie przedstawia dwie tablice z nazwami warszawskich ulic, w tym jedna wykreślona, z napisem "decyzją Prezydenta m-ta po wyroku Sądu Woj. ". Na ul. Jezuickiej zabito Grzegorza Przemyka. W al. Szucha za okupacji mieściło się więzienie gestapo.
