- The front of the card, with stripes in Poland's national colors at the bottom, carries a graphic image in a form of a round watermark, with caption "educators", and a symbol merging a cross and an anchor. On the back there are fields for personal details, and a biblical quote (Psalms 29:11) against the background of Polish national colors.
